A government-appointed review has called for a suite of regulatory responses after finding Singapore-owned Optus failed to grasp the scale and severity of a massive network outage last year.

Star Entertainment executive chair David Foster has made a sudden departure from the troubled casino operator with board colleague Deborah Page accusing him of fuelling an acrimonious relationship with the regulator, the Bell II inquiry has heard.

Lawyers representing four executives working for Super Retail Group claim an external informant has come forward with new details of an affair between its CEO and human resources boss, claiming its board were aware of the secret relationship as far back as December.
