Japan Club: Condition Surveys overview during 2015

The Japan P&I Club has issued a special circular referring to the condition surveys carried out during 2015 with the aim to prevent various types of incidents by checking risk factors, regardless of the impact they may have, that could eventually lead to claims.

ACCIDENTS

 

Bilge cover left open neglecting the oncoming risk

IMCA has reported an incident in which crew, during a routine walk around the vessel, noticed in the engine room that a bilge cover had been left open. There was no barrier or signage in place. On investigation the bilge cover had been opened to permit brine pump maintenance work.

 

PSV involved in a near miss whilst on location

The Marine Safety Forum has published safety alert regarding a near miss in the 500m zone which involved a PSV entering the zone at an excessive speed. The excessive entry speed and direction of approach was observed by the other watch officer on the vessel and also by the crew of the installation.
